Services Supervisor (RN) (Job ID #3272)
This role must be an RN and is responsible for accomplishing the services to H@HC clients and ther duties as assigned. This role reports directly to the Services Manager.
Key Results Area #1 - Fulfill the Client Process (New and Established)
- Intake the client by reviewing the manuals and completing intake forms
- Write the Care Plan according to the county referral and client's desires
- Visit clients according to DHS or HAHC schedule
- Follow-up the intake and/or caregiver placement after 2-3 weeks, then according to evaluation schedule to ensure the plan of care and delivery is met
- Hour Review - ensure cares are being scheduled appropriately
- Update care plans and annual paperwork as needed
- Visit to replace caregivers as needed
Key Results Area #2 - Supervision of Caregivers
- Complete new hire paperwork
- Orientate the caregiver to the Care Plan
- Provide support and develop relationships
- Respond to concerns in a timely manner
- Provide coaching/disciplinary action as needed
- Training/evaluation of tasks and competencies according to DHS or HAHC schedule
Key Results Area #3 - Promote Teamwork
- Incorporate HAHC Vision and Values in work and activities
- Documenting all relevant correspondence
- Work with the other Services Supervisors to become familiar with each other’s clients, caregivers, and concerns, for backup and support
- Actively participate in departmental meetings and projects
- Stay current on HAHC policies and events
- Develop relationships with other HAHC team members
